The PEER Committee makes the background check forms available online for the convenience of the filer. The forms cannot be submitted electronically; they must be printed, signed, dated and notarized.
The forms are in Adobe Acrobat PDF format. They are intended to be completed on a computer using either Acrobat Professional, Acrobat Standard or the free Acrobat Reader. Acrobat Professional and Acrobat Standard have the ability to save the data that is entered. Thus the filer could start completing the forms, save his or her entries, close Acrobat Professional or Acrobat Standard and finish completing the form another day. However, since the form cannot be saved by Acrobat Reader, the filer must complete the form at one sitting or not close the file containing his or her entries until he or she is finished and has printed the forms.
Alternatively, a filer could print the forms and then complete them using a typewriter.
The completed, paper forms must be returned to the appointing authority--e.g., the Governor--rather than to PEER.
